I'm a little confused about what has the text "series_index" in it - is it the Collection name, or the Titles of books in the collection?

How are you creating your Collections? Can you post your Settings data from the report generated when you run Create Collections, and possibly some of the collection names that are generated. If you are just creating from the Series column, with no other settings, then only the Series Name should show up in Collection names.

The plugboard above is only used for the Title of books. So it won't affect Collection names. Once you open a collection you should see a list of your book titles with the series_index shown (and not containing the text "series_index") if you are using the plugboard you listed.

Some more details should help find the issue.
