I know this sounds stupid, but did you check the wall socket? There might be an intermittent problem there.

Also, one frequent problem with the USB chargers is not the charger itself but the attached USB cord. Frequent or too severe flexing can break the tiny wires inside the insulation, with the result that the charge will be intermittent or non-existent. Intermittent could be worse, as there is a slight surge each time the wires within the insulation make contact which could potentially harm the reader. It's like repeatedly plugging it in and out of the power source (which is why I always plug the cord into my reader before plugging it into the power source). If you have an extra cord with the proper fittings lying around, you may want to try that first before replacing the charger, although the chargers are fairly cheap.
