You cannot change the folder/filename Calibre uses internally for storing files - there is a sticky at the top of the Library Management forum area bout this.
In terms of exporting files from Calibre then there are filename masks that can be set under Preferences to control the format of filename generated. For the exported file. You did not mention how you are getting the file out of Calibre, so it is not possible to point you at a specific one.
