Some forum members have reported problems when using Calibre to add books to their Touch. Try using only Adobe Digital Editions or Windows Explorer and see whether that solves the problem.

Also, a factory reset deletes all books from your Touch, except those on a SD card, so that is why you no longer see your library book.
