I did as you suggest and the default layout was restored. With the "Read" column far to the right the data is visible. When I move it to the far left it remains visible until I connect the device and then once again the Read column collapses and when expanded the data is not visible and there is no "On Device" column.

I disconnected the device and reset the default a second time. Without moving the Read column to the left I reconnected the device and I find that the Title column, which is the first column to the left, is almost completely obscured and there is no way to scroll that far to the left so I suspect that the issue isn't with the "Read" column so much as when the device is connected I am not able to view everything that is to the far left. Perhaps the "On Device" column is still there, I just can't see it.

When I go into preferences and click on "Add Your Own Column" I see that "On Device" is still at the top of the list, but it still has no check box.
