I've got accounts in the US, UK and Germany. All link to the very same German credit card.
You may receive a note such as "you've bought lots of books outside of your original region in the last few weeks. Did you move recently?". But still they don't force any account onto you.
I figured eventually they would get fed up of just asking and do something about it I thought I had read in the Kindle forums of people being asked to provide ID to prove nationality/location. I use Entropay and a valid US address where mail would eventually reach me, and don't mind the extra 5% for the peace of mind.
