I actually agree that the Nook should use the 1st page render as a fallback if a better method isn't available. It's annoying for PDFs (since there is no way to attach a cover image), and for books from the library (which I refuse to strip or otherwise tamper with).

Even better would be if ePub 3.0 actually specified something a bit more concrete that would allow someone to set a thumbnail or cover image.
