The wiki table looks good. You asked for comments, so here are a few:

The Notes did not work for me.

I think PRC and MOBI are probably the same format for the purposes of this table, perhaps use both names in the title. Also, Amazon's AZW e-books are MOBI but because of DRM they can only be read on the Kindle. So it is usual to call them AZW and treat them separately. For single-site (or few site) formats like this, it may be best to include an OTHER column with the actual name of the format in the cell.

Baen also provides a HTML version.

Manybooks provides all formats under the sun.

Mobileread uploads obviously has MOBI and IMP.

How should DRM be handled? One approach would be to use X for DRM-free and S (secure) for DRM-laden. Some cells would have both.

Should Secure Adobe e-books be called "PDF"? Perhaps the DRM vs DRM-free notation is enough, but there is way too much confusion about Adobe e-books.

Is the PDB column mostly eReader? A single column for multiple formats may be confusing.
