Oh, the joys of living in S.E. Asia! I used to have all my shirts made for me by this little old man who had a 'garage' shop near my house in Thailand. I'd supply the cloth, (bought at the local fabric shops,) thread (Thai thread is waaaay too weak) and he'd sew up the shirts for about $3 USD each. Considering my neck, shoulders, and chest need an XL, my waist needs an L, and my arms need an M, getting shirts that fit right off the rack has never worked for me! He made all the formal wear for the Japanese women in town too!
