I have tried with MAC filtering off and on, reset the router multiple times and have reset the kobo as well. I have tried manually adding the network, and again got the incorrect login for my network message.

It will connect with no security, so I know the wifi hardware is fine.
There are actually 4 different models of the DIR-615;

DIR-615
DIR-615_REVB
DIR-615_REVC
DIR-615_REVE
You can tell which model you have from the bottom

Where do I locate the serial number (SN), hardware version/revision information (HW ver), MAC address and model number?
The serial number, hardware version/revision information, MAC address and model number, will be located on a label on both the outer box, and on the adapter itself, usually attached to the bottom of the unit.
615 had rev 1.00 and 1.10 firmware
615 B had 2.21, 2.23, 2.24 and 2.2.5
615 C had 3.00, 3.01, 3.10, 3.11NA, 3.12NA, 3.13NA
615 E had 5.00NA, 5.01NA and 5.10NA

Many of these revisions existed to fix compatibility issues with other manufacturers.

The main support page for the DIR-615 is http://www.dlink.com/products/?pid=DIR-615 . On this page you can then click on "Support Resources", then after drilling into your particular version, then click on the Firmware link.

This is not to say it will solve the issues, but, the same way as often the advice for software issues is to ensure you are on current levels, it also applies to the software on routers.
