The meatgrinder is a Word document to ePub via some version Calibre. And as the saying goes, Garbage in. Garbage out. and the Word document is chock full of garbage.

If the OP is here, I'd like the OP to petition Smashwords to allow ePub uploaded directly.

Also, the filenames of the files in the ePub are truly nasty.
