My wife and I were having dinner in one of her favorite restaurants when I felt a small bit of detritus in my mouth after sipping my coffee. What it was, I have no clue, but I'm almost certain that it wasn't food, and I know it had nothing to do with the coffee not it's cup.
When alone, and depending on how egregious it is, I'll tell the waiter/waitress to take the meal away and/or ask to see the restaurants manager.
What do others do? Make a fuss, or keep quiet and let the others enjoy their meal?

Unless it's something horrifying I just keep quiet about it for that exact reason. It spoils everybody else's meal and I hate it when someone does that to me. If I feel like it needs to be brought to a manager's attention I do it discreetly after the meal.
