Hmm try the --verbose switch. Is html2lrf detecting a chapter beginning. If so, you can disable it with --chapter-regex "$"
As for the ideal image size...I'm afraid I don't remember, you'll have to experiment. Use the --lrs switch and look at the LRS file to see if html2lrf is including the original image or not.
