They gave me a "Return Merchandise Authorization (RMA) procedure", but did not supply an authorization number.

SORRY! I don't know *WHERE* I got the clue you were in Australia, but I surely did!
The RMA process *should* have included a number. Email them again and make sure you let them know that you didn't get an RMA number. It should be a string of something like RMA-customername-MMDDYYYY-xxxx.
As for the card reader not formatting the SD card, perhaps it's not being recognized by the computer in the USB port you chose. Please try another.
