With Mobi files, the cover image is considered part of the metadata, and as such the Kindle is not set up to be "opened to" metadata. Their are tricks and hoops to jump through (like adding the cover image as an inline-image in the text) to force it to start at a cover image, but to be honest... the average Kindle user is used to not seeing the cover on books they purchase (unless they really, really want to... and in that case they know how). It's really not worth fighting, but if you insist... there's a few threads in the Mobi forum that discuss workarounds/solutions. I'll see if I can track them down.
