Creating a table of contents

I have tried to go though the instructions on doing so but I am having issues. This was ran through Calibre once and now I want to see if I can create a table of contents. The tutorial mentions
,
, etc. but mine below is
where it has CHAPTER all the time and the chapter number like
1.

Can someone help me out here?
