quite possibly, your Kobo could have had some bad blocks on the internal memory. that would account for the issues you have had sideloading to main memory and unmounting the drives.

a partially corrupted DB could explain it also, or a combination of the 2.

ed: another thought - could possibly be the USB port or cable as well. XP and the power management of USB hosts can get flakey sometimes, i see it at work all the time. I am constantly replacing USB mice & keyboards, just to have them come back and work fine on a different machine.
