You'll find that Calibre will do a very good job, within the limits of the source format. It doesn't support Word DOC as an input format, but resaving in Word to either RTF or "HTML Filtered Web Page" will yield a file that Calibre can work with.

PDF is a horrid source format. You'll want to avoid this format as much as possible. There are almost always flaws in conversions from PDF regardless of the conversion tool used.
