If you're familiar with Amazon's Mechanical Turk, this is the approach they use with worker account suspensions:

Hello,

I'm sorry, but your account was closed due to a violation of our Participation Agreement and cannot be reopened.

Any funds that were remaining on the account are forfeited, and we will not be able to provide any additional insight or action.

Our Participation Agreement outlines the terms of our program and, since our suspension criteria are proprietary, we do not specifically publish or share them....


Thankfully, I haven't received that letter, but that's how you get canned.
