There seems to be a permissions issue with your (hidden) Calibre settings folder/file.
Or
You are restoring this directory between uses.
To locate the Directory:
Open Preferences: Miscellaneous
Click the third button down: "Calibre Configuration Directory"

You must have Owner-Read-Write privileges
