I have seen no evidence that they do. I post and download and connect from a Canadian IP address, but have a US mailbox address that I use for Kindle (and other stuff, since I'm close enough to the border to have stuff shipped there.) The one annoyance is that I get dinged for Washington state sales tax (less than HST, certainly) because that address is in Washington. But that's less of an annoyance than trying to change it.
The one place you WILL have Kindle and Amazon notice your address is with WhisperNet. If you connect from outside the US, you can't get everything free. Magazine subscriptions, for example, require paying an (outrageously high) weekly fee. And you'll get dinged for stuff you send to your kindle email address for automatic whispernet transfer. But as long as you use WiFi, that isn't an issue.
