It's been awhile since anyone posted on this thread, but i'll throw my $.02 in...
I added the following lines in my .opf file in the manifest:
<guide>
<reference href="images/front_cover.jpg" type="cover" title="Cover"/>
</guide>
Once I added them, iBooks automatically found the cover image and made the thumbnail from it.
