Here's exactly what I did (with Word 2007 already installed)...
1. Download and install the Microsoft DHTML Editing Control.

Step 2 is only partly correct. The problem is that the update is incomplete and unless you've gone through the 4.0 install and then ALL the other updates as they came out, you'll have problems. What you can do however to easily solve this problem is go to
https://www.mobileread.com/forums/showthread.php?p=78249 and download the update in the first message which is complete. I made sure it was. Now once you use it to update, it'll be a complete update.
The reason you need step 4 is because you've installed Book Designer in Program Files which is a no-no (according to others) because Book Designer writes to it's own directory in a way that is not correct for Vista. Best to install under the root of C: instead.
