OK, that's the problem then. The mobi format (Kindle) does not use the toc.ncx file from the epub (except to allow the chapter hopping via the 5-way switch). You must create an html file with the table of contents links in it. Calibre will do this for you, but kindlegen will not.

There are several threads dealing with this topic, in the Mobi, epub, and sigil forums. Good hunting!
