You need to not use Calibre for this if you want that level of control. Convert to htmlz or rtf. Then use an editor that lets you control all that like OpenOffice or MS Word. Once you have the page size, margins, headers, footers, etc the way you want just print it to pdf with a pdf printer/distiller.
