The little arrow on the Convert Icon, Create Catalog: Select CSV output. Select the fields to be included on the options tab

I believe the Catalog created (still) respects books that are selected (highlighted).
You will need to create separate files for each storage device on your device (If all the books on your device are shown (device view) as being in your Library, the 'ondevice' column in Library View reflects ALL storage, so that would do the trick with 1 file.
