Can you run in debug mode as shown in Troubleshooting and PM me the output. Add some books, send to device, add a collection name to a book, and then run the Preview. (note your first report looks like Create not Preview).
Aside from the 0 length books, it appears you have 2 collection names for Cornwell, Patricia - one listed as Kindle one as Calibre managed which also has my interest so I'll try to test that.
