No. I just added it with the 0.7.55 release. I got tired of calibre looking out of place on OS X. The menubar is also available to Windows and Linux users. But OS X is the only OS with default items in it.

Not a bug. You have to have Preference in one or both of the toolbar or menubar. If you remove it from both it is automatically added back. This is to prevent you from locking yourself out of accessing the preferences dialog.

The preferences menu item is moved to calibre/Preferences automatically by Qt. There is no way I know of to prevent this as that is where OS X says preferences items should be. I also can't add it back to the preferences menu item because of this.

I need to do some testing to get it looking 100% right on OS X. I need to ensure that OS X users can still get to preferences even when it's removed from the menubar and toolbar.
