It's where the Kindle page counts come from on side-loaded/non-Amazon books. It creates and sends the page count file when the books are transferred to the Kindle from Calibre. I've never actually looked at the page count file, I just pull up the count from within the book once it's on there. It's controlled by a plugin which was added to calibre a while ago (right after the 3.1 firmware was released for K3), and there are 2 settings: a fast option and a more accurate option. I use the more accurate option and it's close enough for me.

There's a thread in the calibre forum somewhere but my fever is affecting my search abilities. Sorry.
