There are two ways to grab the image from the book after you add the book.

Select the book, open the Edit metadata window and click the Brown book icon under the available formats area.

Select multiple books, open the Edit metadata window, under change cover check Set from ebook file, click OK or apply.

If there is a cover within the book it will be placed appropriately in your Calibre library.
