There are always issues with installed extensions when a new release arrives. Some are an easy fix, and some are not. Problems tend to be greater when the upgrade is a major version, as that changes things under the hood that an extension may rely on.
In going from 3.6 to 4.0, I only lost two extensions. One was Doron Rosenberg's GMail Notifier, and the other was Mel Reyes Total Toolkit. GMail Notifier is dead - Rosenberg announced he has ceased development and is dropping support. Reyes promised an update for Tech Toolkit when FF 4.0 got a production release - he didn't feel like trying to hit the moving target a beta represented.
I'm currently running 35* extensions, and all work fine. Some are new to FF 4.
The quick fix for addons problems is to install the Addons Compatibility Reporter. That lets you submit reports on stuff that either still works or is broken, but has the side effect of turning
off version compatibility checks, so FF will enable and attempt to use your addons even if it thinks they
don't work with the new release. Most will.
Every addon has an install.rdf file, and the install.rdf file has minValue and maxValue fileds that specify the oldest and newest versions of FF the addon is certified to work in. In most cases, when you install an upgrade to FF and it thinks existing addons aren't compatible, they actually
work just fine, but FF doesn't know that because the maxValue filed in the rdf file is for an older version.
You can install the Addons Compatibility Reporter or the Nightly Tester Tools addons to turn of the compatibility check, or you can get the XPI file for the archive (it's a Zip file), extract the install.rdf file and edit maxValue with a text editor, then stuff it back in the XPI and install from the local copy. I find it easier to turn off the compatibility check.
