Why not create a new account in your sister's name and re-buy the book (and any other subsequent gift books) for her that way?
It may not be an optimal solution, but it would probably take a while for the Smashwords people to implement what you want.
And in the meantime, your sister would have full access to and be able to download replacement copies of the book and review/add titles to her wishlist for you to buy as future gifts.
