Did you get the book from Kobo, by any chance? I have seen this before, where the margin cut is affected differently on the first page than on the other pages. It was on a Kobo book where margin cut was not working well after the first page. I think epub just has a setting where you can set the margins differently for the first page of a chapter. I just reformatted the problem book in Calibre.
