Also, Mobipocket Creator did a great job extracting the pdf for me - sans page numbers, and the resulting html has chapter headings tagged h2, so I created a very workable TOC. Only glitch is that chapter headings with apostrophes lost them, but I can live with that. (If it bugs me too much I'll ship the html to Sigil and fix it.)

PDFs can be such testy critters. Sometimes calibre works better, sometimes Mobipocket Creator works better, sometimes nothing short of text extraction and heavy editing will work. Fortunately, this particular one was pretty manageable.
