Several possibilities as you suggest... could be caused by ebook problem, have you had calibre and SRL open at same time with reader connected (a real no-no, two programs trying to access same stuff at same time, corruption, data loss almost guaranteed), replacing image file might have caused corruption especially if differing sizes but same name and also relating to previous point (safer to delete file then replace)... sorry can't be more definitive but all these things can cause problems.
