I am new at using calibre. Just finished a conversion but having trouble with creating a TOC. I have an Inline Table of Contents, read through the User Guide, followed the instructions in creating a TOC but not happening! I am on a MAC.

Any input is appreciated. I'm sure I will need more help down the line. It is good to have this forum.
