This wasn't a cheap place either, a so-called "gourmet" pizza restaurant, the bill was just shy of 70 bucks just for the two of us. Nothing like paying good money to get your clothing ruined

Yes, you should have asked to speak to the manager. If the girl was that young, as sad as it sounds, she just may not have known any better or been too embarrassed to say anything. And if management doesn't know it happened then they will never have the chance to correct both the situation and the way she handles it in the future. If you simply left a smaller tip then you went away angry and she got angry and nothing got resolved.
