small images? Were these somewhere on the original PDF page? Try and imagin a PDF page made up of layers. The image was on a different layer from the text (and other images). The order does not matter to a PDF, the 'stack', as viewed is what you get.
Calibre un-stacks the PDF to convert, but has no clue how they were to be ordered visually, when placed end to end
