i have sometimes "lost" header tags by deleting what seems to be superfluous white space in book view, in front of the chapter header text. this can cause h2 tags to revert to normal tags.
check also for nesting. if you mix h1 & h2, then view toc in calibre viewer, to shows the h1 only but with clickable indents which expand to show h2 when clicked
