Don't use PDF as your source file, it will introduce too many mistakes in your epub and mobi files. PDF should only ever be a destination file converted from something else.

Epub and mobi files already contain the cover and any other files they need to work. What you see in the Calibre directory with them is just for the use of Calibre.
