I'm about to send off my claim form to get the VAT back from my purchase of a Sony PRS-650. In order to do that I have to include my European Guarantee Information Document, the problem is I have several Sony products and I've managed to mix up the various forms (doh).

I don't want to send the form for a Sony cover instead of the Reader, so I was hoping somebody could take a look at their PRS-650 European guarantee document and let me know the document number (printed at the top right).
