Adding the cover to calibre is not the same as adding the cover to the book. Covers are only added to the book during conversions. Even a ePub - ePub conversion should set the cover in the ePub and let the tool that creates your missing icon work.

I bet the icon is created on the fly by finding the cover that is embedded in the book. Adding the cover in to the book via conversion to epub should enable this feature to work for all of your books.

Keep in mind that a ePub to ePub conversion overwrites the original ePub so you might want to have a backup in case the converted ePub isn't to your liking. Additionally calibre can not convert books that are restricted by DRM.
