Try eBook publisher to make your books. It builds an intermediate HTML file that we can look at to find out what is wrong. It is a free download from ETI. Actually ebookwise librarian also builds and intermediate file but then erases it. I am not sure how to make it stay around.
eBook publisher gives you the best control over content of any choice out there.
