I don't know what your all caps problem is. The suggestion I made solves the issue of your book titles/authors getting overwritten when you retrieve metadata. If your titles/authors were already formatted and containing the content you want, then unchecking that preference ensures they stay that way, rather than getting overwritten. The "problem" lies in the metadata sources, clearly someone decided (inconsistently) that they like titles to be stored with crap casing rather than how they are presented on the book cover which is how I like them.

As for the It's thing. "It's" was just an example word. As far as I remember the problem presented itself with any word that contained an apostrophe, like "don't" etc. When I enquired on the forums about it a while ago (or a ticket, can't remember which) I was told by Kovid it would never be fixed. If it has indeed been improved in that area then great, I have my own tool to rename files before I import them that does a better job of title casing than Calibre used to so haven't tried it since.
