From the Smashwords publishing support FAQ:

Are there tax withholdings on my earnings if I reside outside the United States?

Main requirement is that you have to have at least $10 of accrued earnings with Smashwords before applying to them for the required letter to accompany your W-7 form to the IRS to apply for an ITIN.
