Apple might not give you free rein to do as you please, but they don't change your formatting. If your EPUB file passes Epubcheck, it goes through their system.
That's quite unlike Smashwords, where you submit a Word doc, and their processor creates the EPUB and MOBI files. With Amazon and Apple, you can submit the MOBI or EPUB file, and as long as it meets their requirements, it will go through unchanged.
