The short answers are:
Yes.
Only if doing a format conversion as well.
No.

The longer answers are:
Yes, for everything except books bought in Apple's iBooks store, as there are no scripts to remove the DRM from them. You need to do a google search for a blog by a chap named Alf, 'Apprentice' to the Dark Reverser. There you'll find links to a zip file of all the tools needed to removed DRM, and instructions in the ReadMes in the archive. A new version 2.3 of the archive has just been released.

Removing DRM doesn't affect the layout of Mobipocket or ePub ebooks. It shouldn't affect eReader ebooks either, although some badly made eReader books might suffer slightly. However, if you also need to convert to a different format (say, using Calibre), then the formatting might change a little.
