If a card is formatted with FAT16 there is a limit to the number of files/folders allowed at the root level. If this is the issue, then the answer is to either format with FAT32 instead, or to move all the files/folders down a folder level so they are no longer at the root level.

It might also be something simpler such as the card having the write-protect switch set.
