That sucks. It sounds like you have done everything possible to try and solve the problem.
Sometimes the only thing bullies understand is bullying. Someone should beat the sh*t out of the bully and see if that helps.
I know, I know violence begets violence, but....
The schools and teachers are often completely restrained by fear of violating the rights of the bullies and they tend to err on the safe side. I think we need to bring paddling back to the schools! And parents must be required to sign a waiver when the child enrolls.
We can't turn responsibility for raising our children and instilling morals and ethics into them over the schools (institutions) without turning over the power of punishment as well.
