I chose the cover option, but that's just initial selection, I've never bought a book solely on the cover. I always read the back blurb, in the past that was usually when I decided to buy or not buy the book. Since I now mostly buy ebooks I usually take a look at Amazon or Goodreads what the average rating of the book is and read a couple of reviews (both positive and negative).

I buy most books from BooksOnBoard and ebooks that doesn't have a cover and/or description I dismiss right away, I might miss some interesting books this way but if the publisher can't be bothered to provide a cover, even if it's a simple one, or a description/synopsis I feel that the book is probably not worth it.

I also read books recommended by people who have about the same taste as me. I probably wouldn't have picked up several series which covers features half naked people, which I also mostly dismiss.

Reviews have become more important to me the last couple of years. Ever since I started buying most of my books online instead of from a brick and mortar store. But since joining Goodreads earlier this year I check reviews for nearly every book I have bought since.
